# Forms and patient guides

> Attach the right forms to each visit, and send patients a guide to what happens next.

## Forms

Each visit has a **Forms** tab with its **Visit documents**. Click **Add** to attach a form from your library.

When you sign a note, forms linked to that note are filled in from it.

Your practice starts with these forms:

* Authorization Request
* Cardiac Clearance
* Ambulatory Surgery Scheduling Form
* Hospital Scheduling Form
* Hospital Physician Pre-Admission Orders
* Home Health Referral
* Referral Request
* CPM Order + PT Protocol
* Implant / Equipment Request
* Anesthesia Clearance Packet

### Add your own forms

Go to **Settings** → **Document Library**, then click **New Document** or **Open designer**. Click **Save draft** while you work, and **Publish** when it's ready to use. The library is in beta.

Archiving a form removes it from the library. Copies already on visits stay.

## Patient guides

After each visit, a patient guide is drafted from the note: what you discussed and the patient's next steps, in plain language.

**1. Open the guide**

Open the note and click **Patient guide**.

**2. Review it**

Edit anything you like. Click **Preview** to see what the patient sees, or **Regenerate** for a fresh draft.

**3. Save**

**Save** publishes the guide to the patient's private page. Nothing is sent yet.

**4. Send it**

Click **Send to patient**, then **Send by email** or **Open text message**. The patient needs an email address or mobile number on file.

If a patient replies with a question, click **Respond**. To take a guide down, click **Revoke link**.

### Set your practice's guide design

Go to **Settings** → **Patient Guide** and click **Create Patient Guide**. Click **Save as current** to use it for new guides.
